Notice of Privacy Practices

As required by HIPAA, INTIMÉ Miami maintains a Notice of Privacy Practices (NPP) that describes in detail how your medical information may be used and disclosed, and how you can access this information. Key points include:

  • Treatment: We may use and share your PHI to provide, coordinate, and manage your healthcare and related services, including consultations with other healthcare providers involved in your care.
  • Payment: We may use and disclose your PHI to obtain payment for services provided, including billing and collections activities.
  • Healthcare operations: We may use your PHI for internal operations such as quality assessment, staff training, compliance programs, and business planning.
  • Required by law: We may disclose your PHI when required to do so by federal, state, or local law.
  • Public health activities: We may disclose PHI for public health purposes, such as reporting communicable diseases or adverse reactions to medications.

A complete copy of our Notice of Privacy Practices is available at our office and will be provided to you at your first visit. You may also request a copy at any time by contacting our Privacy Officer.

Your Rights Under HIPAA

As a patient, you have important rights regarding your protected health information:

Right to Access Your Records

You have the right to inspect and obtain a copy of your medical records and other health information maintained by our practice. We will provide your records in the format you request if readily producible, or in an alternative agreed-upon format. We may charge a reasonable, cost-based fee for copies.

Right to Request Amendments

If you believe that information in your medical record is incorrect or incomplete, you may request an amendment. We will review your request and respond within 60 days. If we deny the request, we will provide a written explanation, and you may submit a written statement of disagreement to be included in your record.

Right to Request Restrictions

You may request restrictions on certain uses and disclosures of your PHI. While we are not required to agree to all restriction requests, we will carefully consider each one. We are required to agree to a restriction request if the disclosure is to a health plan for payment or healthcare operations purposes and the PHI pertains to a service you paid for in full out of pocket.

Right to Confidential Communications

You may request that we communicate with you about health matters using a specific method or at a specific location. For example, you may request that we contact you only by mail or at a particular phone number. We will accommodate reasonable requests.

Right to an Accounting of Disclosures

You have the right to request a list of instances in which we have disclosed your PHI for purposes other than treatment, payment, healthcare operations, and certain other activities. The first accounting within any 12-month period is provided free of charge.

Right to a Copy of the Notice of Privacy Practices

You have the right to obtain a paper copy of our Notice of Privacy Practices at any time, even if you previously agreed to receive it electronically.

Right to File a Complaint

If you believe your privacy rights have been violated, you have the right to file a complaint with our practice and/or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.

How We Use and Disclose PHI

INTIMÉ Miami uses and discloses your protected health information only as permitted or required by law. Common situations include:

  • For your treatment: Sharing information with physicians, specialists, or other healthcare professionals involved in your care
  • For payment: Submitting claims or providing information necessary for reimbursement of services
  • For healthcare operations: Conducting quality assessments, audits, staff training, and compliance activities
  • As required by law: Complying with court orders, subpoenas, or mandatory reporting requirements
  • For public health and safety: Reporting as required to public health authorities or to prevent serious threats to health or safety
  • With your authorization: For any purpose not listed above, we will obtain your written authorization before disclosing your PHI

We will never use or disclose your PHI for marketing purposes or sell your information without your explicit written authorization.

How to File a Complaint
