You have the right to inspect and obtain a copy of your medical records and other health information maintained by our practice. We will provide your records in the format you request if readily producible, or in an alternative agreed-upon format. We may charge a reasonable, cost-based fee for copies.
If you believe that information in your medical record is incorrect or incomplete, you may request an amendment. We will review your request and respond within 60 days. If we deny the request, we will provide a written explanation, and you may submit a written statement of disagreement to be included in your record.
You may request restrictions on certain uses and disclosures of your PHI. While we are not required to agree to all restriction requests, we will carefully consider each one. We are required to agree to a restriction request if the disclosure is to a health plan for payment or healthcare operations purposes and the PHI pertains to a service you paid for in full out of pocket.
You may request that we communicate with you about health matters using a specific method or at a specific location. For example, you may request that we contact you only by mail or at a particular phone number. We will accommodate reasonable requests.
You have the right to request a list of instances in which we have disclosed your PHI for purposes other than treatment, payment, healthcare operations, and certain other activities. The first accounting within any 12-month period is provided free of charge.
You have the right to obtain a paper copy of our Notice of Privacy Practices at any time, even if you previously agreed to receive it electronically.
If you believe your privacy rights have been violated, you have the right to file a complaint with our practice and/or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
